Immunization Requirements

    • The Conscientious Objector form must be notarized and returned to the LCWM School Health Office before the start of the school year. Our district secretary, Karissa Hall and our Elementary secretary, Megan Hutchens are both Notaries. Please call ahead to schedule an appointment with either of them.

  3. Medical Exemptions:

    • Students with medical exemptions must have a signed form from their doctor and parent, stating the exemption for medical reasons.

  4. Grace Period for New Students:

    • New students are allowed a 30-day grace period to provide proof of immunizations or objections.

  5. Deadline for Homeschooled Children:

    • Homeschooled children have until October 1st to show proof of immunizations or objections, and this must be submitted to the District Office.

  6. Assistance for Parents:

    • Parents facing challenges in obtaining copies of their child's immunization records are encouraged to contact the Nurse for assistance.
